APPLICATION FOR PENSION

To the Honorable County Court of Scott County,
Sitting as a Pension Board: That he is a bona fide citizen of the State of Arkansas and has resided within this State for more than twelve months next before the filing of this application;
That he enlisted as a soldier in the State of Georgia during the war between the States in Company "I" of the 60th Regiment of Ga. Vols.
That while in the discharge of his duty as a soldier of said company and regiment in the service of the State of Arkansas, or of the Confederate States of America, he received a wound, from the effects of which he lost partially the use of his left shoulder, the ball lodging in the bones of shoulder, and the ball was taken out by the Doctors - the sound stiffened left arm and from the effects of which I have always been hindered from work.
That by reason of said wound and disability he is now indigent and incapacitated for manual labor;
That he is not receiving any aid from this State or from the United States, for such injuries, under any other statute, and that therefore he makes application for relief under the act aforesaid.

Duncan, Clerk, sitting as a pension Board for Scott County, do certify that we have examined the application of the within named Thomas J. Farnsworth for pension, under act of the General Assembly of the State of Arkansas, approved April 1, 1891, and the proof in support of same, and find that said applicant is a wounded Confederate soldier, is in indigent circumstances and partially incapacitated for manual labor, and that his claim is just, and that he should be allowed a pension and we hereby allow the sum of thirty dollars per annum.

I, I.K. Leming a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with Thos. J. Farnsworth of GreenRidge, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his wound and find: State description and character of wound: gunshot wound of the left shoulder joint resulting in stif joint
Physical condition and to what cause is his incapacity for manual labor attributable: lame shoulder from wound and Rheumatism of chronic form
Financial condition: Owns a small farm and has no other resources but his own manual labor
Means of support: Labor on farm
Extent of disability: three fourths

Farnsworth, an applican for pension under the Statutes of Arkansas, and that he is incapacitated for manual labor by reason of a wound in the left shoulder, and by reason of said wound he is in indigent circumstance.

I, J.D. Cox, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with T.J. Forrensworth [sic] of Scott Co., Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his wound and find: him wounded
State description and character of wound: his wound is in his left shoulder his arm is stiff to some extent from the wound and according [to] my judgment causes rheumatism.
Physical condition and to what cause is his incapacity for manual labor attributable: According to my notion his wound is one great cause of his disability
Financial condition: he is a very poor man only has a small farm and very little stock of any kind
Means of support: farming on a small scale, hardly makes a support for his large family
Extent of disability: he is not able to work any to amount to any thing but yet he does a little work

On this day personally came before the undersigned a Clerk Circuit Court within and for the County of Scott and State of Arkansas P.J. Bird citizens of Waldron, Scott Co., Ark. whom I certify to be a credible person and worthy of confidence, who being duly sworn states that he is personally well acquainted with the applicant Thomas J. Farnsworth and have known him 40 years. That the material facts set forth in his application for pension are true. That he was a Confederate soldier. Belonged to Company I of 60th Regiment of Ga. Infty. That he was wounded in said service at or near Harrisonburgh, Va. in left shoulder by gun shot wound.
That this information he derived from the following sources--That I was a member of the same Regiment but not the same Company. I was a member of Co. "C" 60th Ga. Regt. was with soldier all the time in the service until the latter part of the war. Was with him 3 yrs. & 6 mo. Was not with soldier however when he was wounded but know he was wounded in the service. I saw him before he was wounded and also afterwards and saw his wounds while he was in the service.
That he is partially incapacitated for manual labor, that I have no interest in this claim.

ADDITIONAL AND SECOND
APPLICATION FOR PENSION

To the Honorable County Court of Scott County:
The undersigned applicant for a pension under an act of the General Assembly of the State of Arkansas, approved April 1st, 1891, and amendments thereto, being duly sworn, states that his name is Thomas J. Farnsworth, his age is 64 years, that he has resided in this State for 23 years, that he is not receiving aid from the State of Arkansas or any other State or government for injuries received in the war between the States.
That he enlisted at Lafayette, Walker Co., Ga. on or about the 12 day of May 1862, in Company I, commanded by Frank Farris in the 60th Regiment of Ga. Infty. commanded by Col. W.H. Berry in Confederate State Service. That while a member of said Company in said service and while in the line of duty at or near Harrisonburg, in the State of Virginia, on or about the 7 day of July 1864 he received a gun shot wound.
Gun shot wound in left shoulder and has during the whole time since at times given me great pain.
My means of support are about as follows: (State fully)
Make my living by farming but don't do any hard labor myself but depend on orphan boys living with me to work on the farm.

On this day personally came before the undersigned a Clerk Circuit Court within and for the County of Scott and State of Arkansas, J.M. Bird, citizen of Waldron, Scott Co., Ark., whom I certify to be credible persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with the applicant, Thomas J. Farnsworth and have known him 35 years.
That the material facts set forth in his application for pension are true. That he was a Confederate soldier. Belonged to Company I 60th Regiment of Ga. Infty. That he was wounded in said service at or near Harrisonburg, Va. in left shoulder.
That this information I derive from the following sources: from long and intimate acquaintance with said soldier and having often heard him talk of his army service and of the wound received as aforesaid.
That he is partially incapacitated for manual labor, and that we have no interest in this claim.

APPLICATION FOR PENSION

I, Thomas J. Farnsworth do solemnly swear that I served as a soldier in the army of the Confederate States, being a member of Co. "I" in 60th Regiment of Infantry Vols. from the State of Georgia, that I enlisted in said service as well as I now remember about May 12, 1862; that I was honorably released from such service on or about the month of March 1865 and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of this State; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life, and that such incapacity (or disability) is the result of wounds received in the service, being gun shot wound in left shoulder causing stiffness of left arm, and from old age, my age now being 72 years, and feeble health, kidney disease, and disease of stomach, rheumatism, and general debilitated physical condition and that such disability is not the result of my own vicious habits still persisted in, so help me God.

EVIDENCE OF PHYSICIAN

I, L.D. Duncan a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with Thomas J. Farnsworth of Greenridge, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition, and find: State description and character of wound or disability: Applicant was shot four times during war. Shot at Fredrick City, Md. in right shoulder joint which has resulted in partial [illegible]. He also has dyspepsia of five or six years standing.
Physical condition and to what cause is his incapacity for manual labor attributable: Senile debility and dyspepsia either of which totally incapacitate him for manual labor and that said disability is not the result of his own vicious habits still persisted in. Have known applicant for 20 years and know that disability is not due to vicious habits
Extent of disability: Total

Harvey citizens of Ark., whom I certify to be creditable persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with applicant for pension Thomas J. Farnsworth and have known him for 28 and 15 years, respectively.
That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age and feeble health, and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge, all property now owned by him and his wife, together, is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That we have no interest in this claim.

Farnsworth and have known him for 40 years and 40 years and about 40 years, respectively.
That he was a Confederate soldier. Belonging to Company "I" 60th Regiment of Ga. Infty. Vols.; That as such soldier he served from about May, 1862 to the close of the war in Spring of 1865. That he was honorably (paroled or released) from such service, and did not desert the same. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of old age and feeble health, and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That we have no interest in this claim. Affiant James M. Bird says he served in same Company and regiment with soldier Thos. J. Farnsworth, and affiant P.J. Bird says he served in same Regiment, and both affiants say they know soldier served as a soldier as claimed and that he made a good faithful soldier and affiant L.W. Dunn says he does not know exactly how long the soldier Thos. J. Farnsworth served in the army, but that he (L.W. Dunn) served in same company and Regiment with soldier Thos. J. Farnsworth and was in several engagements with him and knows the said Thos. J. Farnsworth made a good faithful soldier.
