APPLICATION FOR PENSION

STATE OF ARKANSAS,}
County of Scott}
I, Carey M. Vise, of Rich Mountain, Ark. do solemnly swear that I seved as a soldier in the army of the Confederate States, being a member of Capt. Robt. Boyce's Company of McBeth's Light Artillery of Artillery Vols. from the State of South Carolina. That I enlisted on the 13th day of Sept. 1861 in said service; that I was honorably released from such service on or about the 25th day of April, 1865, and did not desert the same; that I am now, and for the past twelve months have been, a bona fide resident of ths state; that I do not myself, being now a widower, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor have I conveyed title to any property to enable me to draw a pension, and that I am not in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life on account of being blind in left eye and partially blind in right eye and that such incapacity (or disability) is the result of being blind in left eye as aforesaid and partial blindness of right eye, and rheumatism, and from advanced age, my age now being 55 years, and debilitated physical condition in consequence of such rheumatism and almost total loss of eyesight and that such disability is not the result of my own vicious habits still persisted in, so help me God.

We, the undersigned, sitting as a Pension Board for Polk County, do certify that we have examined the application of the within named C.M. Vise for pension, under Act of the General Assembly of the State of Arkansas, as approved March 11, 1901, and the proof in support of same, and find that said applicant is not a wounded Confederate soldier, is in indigent circumstances, and wholly or partially incapacitated for manual labor, and that his claim is just, and that he should be allowed $50.00 pension.

I, I.K. Leming a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with C.M. Vise of Ritch Mountain, Arkansas, who is an applicant for a pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find: State description and character of wound: Disability Total loss of Left eye and Chronic Rheumatism.
Physical condition and to what cause is his incapacity for manual labor attributable
Total loss of Left eyesight and Diseased right eye and chronic Rheumatism and that said disability is not the result of his own vicious habits still persisted in.

On this day personally came before the undersigned, a Circuit Clerk within and for the County of [Blank] and State of Arkansas, J.O.A. Sullivan and M.F. Simmons citizens of Scott County, Ark. whom I certify to be creditable persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with applicant Carey M. Vise, for Confederate Pension and have known him for 20 years, respectively.
That he was a Confederate soldier, as we are informed and believe. That he is now and has been for the past twelve months a bona fide resident of Arkansas. That he is incapacitated for manual labor by reason of almost total loss of eyesight and rheumatism and that such incapacity (or disability) is not the result of his own vicious habits still persisted in. That to the best of our knowledge, all property now owned by him, he being a widower, is not worth exceeding $400 (exclusvie of household goods and wearing apparel). That he is not in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That we have no interest in this claim.

Craddock citizens of said County & State whom I certify to be creditable persons and worthy of confidence, who being duly sworn, state: That they were each, personally, well acquainted with applicant C.M. Vise and knew him six years, respectively. That he is now and has been for the past twelve months, a bona fide resident of Arkansas. That to the best of our knowledge, all property now owned by him and his wife together is not worth exceeding $400 (exclusive of household goods and wearing apparel). That neither he nor his wife is in receipt of any income, annuity, pension or wages for any services, or the emoluments of an office, in excess of $150 per year. That we have no interest in this claim.

On this day personally came before the undersigned, a Notary Public within and for the County of Union and State of South Carolina, H.F. Scaife and J.K. Young, citizens of Union, South Carolina, whom I certify to be creditable persons and worthy of confidence, who being duly sworn, state that they are each, personally, well acquainted with applicant Carey M. Vise and have known him for 40 years, respectively.
That he was a Confederate soldier. Belonging to Capt. R. Boyce's Company of Macbeth Light Artillery of South Carolina Vols. That as such soldier he served from September 1861 to 25th of April, 1865. That he was honorably released from such service, and did not desert the same. That we have no interest in this claim. That deponent H.F. Scaife was a first lieutenant of said MacBeth Light Artillery, and that deponent J.K. Young was a sergeant of the same.

I, C.M. Vise do solemnly swear that I am now, and for the past twelve months have been, a bona fide resident of this State; that I am now drawing a pension as an ex-Confederate soldier, under an Act of the General Assembly of 1901, having made my original application from the county of Polk; that I am still in indigent circumstances; that I do not myself, nor does my wife, nor do we both together, own property, real or personal, or both, or money or choses in action in excess of the value of $400.00 (exclusive of household goods and wearing apparel), nor has either of us conveyed title to any property to enable me to draw a pension, and that neither I nor my wife is in receipt of any income, annuity, pension or wages for any services, the emoluments of an office, in excess of $150.00 per year; that I am incapacitated to perform manual labor in any of the ordinary avocations of life; that my disability has increased, and I am less able to perform manual labor than when my original application was made, by reason of increased General disabilities on account of old age. That I am now a resident of Scott County and my post office address is Waldron, Ark. and I hereby make application for an increase of pension, under Act of the General Assembly of 1907.

We, the undersigned, sitting as a Pension Board for Scott County, do hereby certify that we have examined the application of the within named C.M. Vise for increase of pension, under Act of the General Assembly of the State of Arkansas, approved March 9, 1907, and the proof in support of same, and find that said applicant is less able to perform manual labor than he was at the time a pension was granted him, and that he is entitled to have the same increased to $100.00 Dollars.

I, Cheves Bevill, a duly registered and practicing physician in Scott County, Arkansas, do hereby certify that I am personally well acquainted with C.M. Vise of Waldron, Arkansas, who is an applicant for an increase of pension under the Statutes of Arkansas.
That at his request I have made an examination of his physical condition and find:
State description and character of wound: No wound
Physical condition and to what cause is his incapacity for manual labor attributable
General debility. Chronic diarhea and hernia (Inguinal). His bowels come down, to a considerable degree, and has never been so that he could wear a truss, and that said disability is not the result of his own vicious habits still persisted in. No.
