Membership Classifications: (check one)

______Annual Membership annual dues $20 per household

______Contributing Membership annual dues $50 or above for an individual, business, or

             civic organization

______Life Membership a one time payment of $200 per member for individuals only

Society Dues are assessed annually per calendar year of January – December.

Payment of dues entitles a member to the following

• to have one vote at all general membership meetings
• to have the right to attend Board of Director meeting and participate in debate
• to run for elected office
• to serve on Standing and Select committees
• to receive four quarterly publications of the Izard County Historian

           The purposes of the Society are charitable, scientific and/or educational within the meaning of Section 501 (c)(3) of the Internal Revenue Code of 1986.  Membership dues are tax deductible.
