This Register contains diploma and certificates from medical schools, sworn statements of the Marion Co Medical Board of Examiners and other types of sworn statements.
    Within the certifications are references to an Act to regulate the practice of medicine and surgery. This act established three boards of State Medical examiners and provided: Every person now practicing medicine in this State shall, within ninety days after this Act takes effect, prepare a written statement giving his name, post office address and county; when and where he received authority to practice medicine in this State; where his diploma or certificate is on record; and, if a diploma, from what school or medical college issued; such statement shall be sworn to before some officer authorized to administer oaths, and shall be forwarded to the secretary of the board representing his school of medicine. If it shall appear from such statement that such person was regularly authorized to practice medicine under the then existing laws, the said board shall register the name of such person in their list of accredited physicians, and issue to such person a certificate that his name has been placed upon such list."     The certificates recorded in this book are those certificates. I contacted the AR Medical Association and was told they no longer have these applications and don't know of any place they can be found.

"We the undersigned members of the Marion County Medical Board of Examiners hereby certify that we have on this day examined B L Elam touching his qualifications to practice medicine and surgery and pronounce that he is qualified to practice the same under the Laws of the State of Ark." 6 Jul 1897. Isaac W McCain Doctor of Medicine. Ordo Medical College, Milwaukee, Wisconsin. (This Diploma copy is in Latin, including the name and location of the college. This is the best translation of the name & location I could find on the web. There is no date listed in the Diploma.) Recored 9 Oct 1899 in Yell County, Book A, page 214. Albert Elton Certification from Marion Co's Board of Medical Examiners signed by F G Huddleston, J I Thompson, S L Brooksher. "Know ye that Albert Elton has this day been faithfully and impartially examined by the undersigned Board of Medical Examiners of Marion County, in the state of Arkansas ... and has satisfied said Board that he is twenty-one years of age, of good moral character and duly qualified in knowledge and capacity to practice medicine and surgery and is therefore entitled to registration." 26 Mar 1903. M D L Mathews Certification from Marion Co's Board of Medical Examiners signed by F G Huddleston, J I Thompson, S L Brooksher. "We the undersigned do hereby certify that we have this day examined M D L Mathews touching his qualifications to practice surgery and finding him qualified to practice same. G F Elam Certification from The State Medical Board of Arkansas Medical Society,(names of Board are listed) Little Rock, AR. No. 2818. "To Whom it may Concern: A sworn statement having been presented to this Board that on and before May 17th 1903 G. F. Elam was legally qualified to practice medicine. Therefore in accordance with Section 6 of "An Act to regulate the practice of Medicine and Surgery approved February 17th 1903" and effective ninety days thereafter this Certification is issued to him attesting the entry of his name upon the register of accredited Physicians. In testimony whereof are hereunto affixed our signatures and the seal of the Board this eighteenth day of August 1903." B F Gray, DDS Certificate of Registration. "The Board of Examiners, State of Arkansas, hereby certify that B F Gray DDS has complied with the provisions of the law of this state regulating the Practice of Dentistry. A Graduate of Indiana Dental College and is therefore entitled to all privileges therein granted. Given under our hands and seal this 5th day of December 1898." George B Snyder Temporary Permit, State of Arkansas. No. 17. "The Board of Examiners hereby grants to Geo B Snyder a temporary permit to practice Dentistry until the next regular meeting of the Board which will be held on 7 & 8 days of May 1906. After the examination of the date herein this permit is null and void. Given under my hand and seal this 8th day of Feb'y 1906." (signed) A T McMillin, Sec. recorded Feb'y 14th 1906. (top page 31)

F M Phillips Certification from The State Medical Board of Arkansas Medical Society,(names of Board are listed) Little Rock, AR. No. 653. "To Whom it may Concern: A sworn statement having been presented to this Board that on and before May 17th 1903 F. M. Phillips was legally qualified to practice medicine. Therefore in accordance with Section 6 of "An Act to regulate the practice of Medicine and Surgery approved February 17th 1903" and effective ninety days thereafter this Certification is issued to him attesting the entry of his name upon the register of accredited Physicians. State Medical Board, certify that James Adams appeared before us and was duly examined as required by law and was found to be qualified to practice Medicine, Surgery and Obstetrics; therefore, by virtue of Authority vested in us by an Act of General Assembly of the State of Arkansas to regulate the Practice of Medicine and Surgery, approved February 17, 1903, we issue to him this Certificate entitling him to register as an accredited Physician. "Pursuant to "An Act to regulate the practice of Medicine and Surgery approved February 17th 1903." G R Thompson has passed a satisfactory examination before this Board and has complied with all the requirements of the law. He is therefore deemed qualified to practice Medicine and Surgery and this Certificate is issued attesting the entry of his name upon the Register of Accredited Physicians this 12 Apr 1905." P H Keefer "We members of the State Medical Board, certify that P H Keefer appeared before us and was duly examined as required by law and was found to be qualified to practice Medicine, Surgery and Obstetrics; therefore, by virtue of Authority vested in us by an Act of General Assembly of the State of Arkansas to regulate the Practice of Medicine and Surgery, approved February 17, 1903, we issue to him this Certificate entitling him to register as an accredited Physician. C. West Certification from The State Medical Board of Arkansas Medical Society, Little Rock, AR. "A sworn statement having been presented to this Board that on and before May 17, 1903 C. West was legally qualified to practice Medicine, therefore accordance with in Section 6 of "An Act to regulate the practice of Medicine and Surgery approved February 17th 1903" and effective ninety days thereafter this Certificate is issued to him attesting the entry of his name upon the Registry of Accredited Physicians this 18th day of August 1903." Enoch Still "I Enoch Still do swear that I am the keeper of a Grist Mill that grinds for toll known as the Still Mill on Water Creek, and that I hereby make application to have my name recorded as a public miller, said mill is situated on Water Creek in Marion County, Arkansas. James E Orcutt Certification from The State Medical Board of Arkansas Medical Society, Little Rock, AR. "Pursuant to "An Act to regulate the practice of Medicine and Surgery approved February 17th 1903." James E Orcutt has passed a satisfactory examination with all other requirements of law. Jasper Thomas Mattthews No. C-327: Certificate of Registration, State of Arkansas. The Board of Examiners. "Under and pursuant to the provisions of "An Act to regulate the practice of Medicine and Surgery approved May 6 1909, this board hereby authorizes and licenses Jasper Thomas Matthews to practice Medicine and Surgery in the state of Arkansas. He has complied with all the requirements of the law and this Certificate is granted non examination and is issued attesting the entry of his name upon the Register of Accredited Physicians."
